Submitting the form will automatically send it to the registrar. In the Graduate Certificate Program four-letter code field, please enter Z016.\u00a0Use a separate form for each degree or certificate you are applying for.<\/li>\n<li>By the end of the Add\/Drop period, please complete the <a href=\"http:\/\/msmc.umd.edu\/wp-content\/uploads\/2023\/02\/apf_masters_and_certificates_202205.pdf\" target=\"_blank\" rel=\"noopener\">Program Completion Application Form<\/a>. Please see<a href=\"http:\/\/msmc.umd.edu\/wp-content\/uploads\/2023\/02\/pre-filled_masters_and_certificates_2020.pdf\" target=\"_blank\" rel=\"noopener\"> an example of how to complete the program information section here<\/a>. Please send this form to Neil Dhingra (ndhingra@umd.edu) to sign and submit. The program code is Z016.
